1. What insurance plans do you accept?

2. Are you in-network with my insurance?

We work with a variety of insurance providers to make our services accessible. Since network coverage can vary, we recommend contacting your insurance provider directly to confirm if we are in-network with your specific plan.

3. What do I need to bring to my first appointment?

Please bring your photo ID, insurance cards, and a list of your current medications.

To save time during check-in, you can also complete our New Patient Forms here before your visit. Once completed, you can bring them to your appointment.

4. How can I schedule an appointment?

To schedule an appointment, please call one of our office locations directly. At this time, appointments can only be scheduled by phone. You can find the phone numbers and addresses on our locations page.

5. What hospitals do your doctors have privileges at?

Our doctors have privileges at several hospitals, allowing them to provide care and perform procedures across multiple locations when their patients need them. These include Corewell Health (with locations in Dearborn, Taylor, Trenton, and Wayne), Henry Ford Health Wyandotte, Select Specialty Wyandotte, Promedica in Monroe, and DMC System Detroit.

6. I was admitted to the hospital. Can my cardiologist see me?

When you're admitted to the emergency department for heart-related symptoms, you’re typically examined by the on-call cardiologist. We understand that hospital visits can be overwhelming, and sometimes our patients may feel more comfortable seeing a cardiologist who’s already familiar with their health.

If you or a loved one would like your specific cardiologist to be consulted, you have the right to request them by name. Once you make this request, the affiliated hospital is obligated to page your doctor. If they are nearby and available, they will come see you. If not, one of their partners will.

7. How can I request my medical records?

To request a copy of your medical records, please contact our office. Processing may take up to 14 business days once we receive your request.

Please note a signed authorization to disclose your Protected Health Information (PHI) is required and can be submited directly through our website.

 

8. How can I request paperwork to be completed?

If you have paperwork that needs to be completed, such as FMLA, DOT, short-term/long-term disability forms, or other cardiac-related documentation, please let us know in advance. You can bring the paperwork to your appointment or send it to us via fax.

For paperwork unrelated to your heart, we recommend contacting your primary care physician, as they are best equipped to handle it. Be sure to include all necessary details so we can process your request efficiently. Please allow 7–14 business days for completion.

9. How do I get cleared for an upcoming surgery or procedure?

For some procedures, a specialist may require clearance from a cardiologist to ensure it’s safe for you to proceed. If you need clearance, please let us know as soon as you schedule your procedure. Adequate time is needed to review your case thoroughly, and waiting until the last minute might mean having to reschedule your procedure.

If you haven’t been seen in our office within the past year, you’ll need to schedule an appointment, as we cannot clear you without a recent visit. If you already have an appointment, please let us know you’ll need clearance during that visit, as this is the best time to address it—even if the exact date of your procedure hasn’t been scheduled yet.
