Patient Assistant Programs

Patient assistance programs (PAPs) are sponsored by pharmaceutical companies to help patients access their medications at little or no cost. Each program has its own eligibility rules, but don’t worry—that’s where I come in. I'm here to help you navigate these programs and take the burden off your shoulders.

Simply download the forms below, fill out the required sections, and when you're ready, click the "Submit Forms" button at the end of the page to securely upload your documents. If you prefer to fill out the forms by hand, you can print them out and bring them to the office in person. I’ll handle the submission and keep you updated on your application status.
